Referring a patient is one fax

Send your referral to our fax line and we handle the rest: our office contacts the patient directly to schedule, so your staff doesn't have to follow up. Once the patient is seen, your office receives the results by fax — closing the loop on your order without another phone call.

What to include with your referral

A demographics face sheet from your EMR plus a copy or listing of the patient's insurance covers everything we need. Specifically:

• Patient demographics — name, date of birth, address, and phone numbers (a cell number helps us reach the patient fastest).
• Insurance information — carrier, member ID, and group number for primary and any secondary coverage.
• Reason for referral — hearing loss, hearing evaluation, tinnitus, dizziness/balance, cerumen concern, or the relevant portion of your note.
• Your office's information — referring provider name and the best fax number for reports back to you.

Missing something? Send it anyway. If we need anything else to get the patient scheduled, we'll fax your office a short, specific request rather than letting the referral sit.

What your office can expect back

• We contact the patient — not your staff. Our office reaches out directly by phone to get the patient scheduled at whichever of our three offices is most convenient for them.
• A scheduling confirmation to your fax. When the patient books, your office receives a faxed acknowledgment with the appointment date, so you know the referral landed and is moving.
• Results faxed back after the visit. Following the evaluation, Dr. Pitt's report is faxed to your office — our goal is within three business days of the appointment.
• A note if we can't reach the patient. If repeated attempts don't connect, we fax your office an unable-to-reach notice so the loop is never left open.

About the practice

Hearing-Doctors is a family-owned audiology practice that has served Georgia since 1959, now in its third generation under Dr. Gabriel Pitt, Au.D., a Doctor of Audiology trained at the University of South Florida. We provide diagnostic hearing evaluations, tinnitus and dizziness/balance assessment, and hearing aid care from offices in Thomson, Swainsboro, and Hazlehurst. Dr. Pitt is a participating provider with Medicare, Blue Cross Blue Shield, and many other plans, and we file insurance claims for patients as a courtesy. Your patients see the same doctor of audiology at every visit — and your office gets a report back every time.
