Hearing Disability Assessment

Before you book — please read these requirements:

1. Bring a photo ID. A government-issued photo ID is required at your assessment.
2. No loud noise before your assessment. You cannot be around loud noise prior to your test. Longshoremen (ILA members): you must be clocked out of work a minimum of 14 hours before your appointment time.
3. Payment: We cannot bill or charge health insurance for this assessment. The assessment is $600, paid out of pocket. We accept cards, checks, and cash. A $30 processing fee is added for payments made by card (credit or debit) or check — no fee for cash payments.
