Hearing Disability Assessment. We cannot bill health insurance for these assessments. We accept all major credit cards, checks and cash payments ($30 additional processing fees for check and card - no fees for cash payments). Bring a photo ID. You cannot be around loud noises within 14 hours of your scheduled test. If you work the day before you are scheduled, you must be clocked out with more than 14 hours before your assessment.
